SIPI Child Care Center is taking applications for children 3-5 years old. Program will offer full day option (7:30 a.m. - 4:30 p.m.).
Please bring the following documents to determine eligibility:

• Proof of income for the past 12 months (2010 Income tax returns, check stubs, financial aid statement)
• Child's birth certificate
• Up-to-date immunization record
• Letter of enrollment verification (SIPI students)
• Medicaid or insurance card
• Names, addresses and telephone numbers for 3 emergency contact persons (over the age of 18)
