Some Southwestern Indian Polytechnic Institute (SIPI) work-study students must complete a federal background check. If your position requires viewing Personally Identifying Information (PII), you must submit and successfully pass a federal background check.
Please fill out forms and submit to the SIPI Human Resources Office for processing. A no-cost Notary is available in the SIPI Human Resources Office. The required forms are in a packet below:
